• Valid during clinic operating hours only.
• Prior appointment is required for eye screenings.
• Individuals aged 16 and above will be considered adults. Children’s eye screening is only applicable to individuals below the age of 16.
• If only one person attends the appointment, the combined screening rate of $95.92 will still apply.
• Payments made are strictly non-refundable.
• Only one appointment change is allowed.
• Screenings must be completed by 6 July 2025. No extensions will be granted.
